Our client is one of the leading health care groups and is looking for a Nurse Co-Ordinator needed In Tokai Sub Acute

We are seeking an experienced Nurse Co-Ordinator at Tokai Estate. These 60 beds fully accredited sub-acute facility provides high quality care to patients of all ages.

The Nurse Co-Ordinator will be required to facilitate the safe handover, assessment, admission, transfer or discharge of all patients. They are responsible for patient safety and will ensure key decisions are made.

To provide safe and quality treatment to all patients, as well as assisting the multidisciplinary team to ensure effective and safe treatment of patients.

The incumbent fosters the relationship between the patient and the facility to provide excellent care. The Nurse Co- Ordinator also plays a large role in communicating with the families and interacting with a wide range of staff members.

Duties & Responsibilities:

Daily Doctor’s Rounds

  • Providing a brief overview of the patient, i.e. progress, vitals, wound, mobilization, emotional
  • Record round on ECP
  • Add new instructions to care plan or adjust accordingly
  • Arrange new scripts from pharmacy
  • Ensure medication orders are updated on ECP
  • Ensure all new Doctor’s orders performed

Leadership, Organization and Co-ordination

  • Identify potential patient safety issues prior to admission to the facility as part of preparing for the client’s arrival
  • Work closely with Unit Manager to ensure that patients are allocated to the most appropriate assessment
  • Co-ordinating the admission and discharge planning of the patient
  • Ensure that clients are discharged safely
  • Ensure the clinical team is delivering the highest level of care to our patients
  • Attend morning handover to keep up to date with what had happened with client overnight or weekend
  • Communicate effectively with all members of the multidisciplinary team and other facilities for the handover, admission, transfer and discharge of clients
  • Communication between members of the multidisciplinary team (MDT) and patient’s family to ensure that everyone is aware of the progress of the patient and the plan forward
  • Facilitate safety and high-quality patient care through clinical judgement/decision making in collaboration with the multidisciplinary team
  • Judge clinical appropriateness of patient referrals and liaise with the doctor prior to admission
  • Build long term bonds with clients and their families for future referrals
  • Ensure written and verbal handover takes place from the acute hospital to sub-acute and asks key safety questions prior to transfer
  • Decide best clinical placement for client – taking into consideration infection control and diagnosis
  • Preparing results and relaying to Doctor and uploading electronically for all medical staff to view
  • Receives updates of patient’s condition from handover (night to day) and the other members of the multidisciplinary team

Communication

  • Meeting with the MDT and holding family meetings for constant flow of effective communication
  • Liaison between the family and the MDT team for continuity and constant communication to the family

Clinical Decision-Making

  • Facilitate safety and high-quality patient care through clinical judgement/decision making in collaboration with the multidisciplinary team

Referrals

  • Judge clinical appropriateness of patient referrals and liaise with the doctor prior to admission
  • Build long term bonds with clients and their families for future referrals

Handover

  • Ensure written and verbal handover takes place from the acute hospital to sub-acute and asks key safety questions prior to transfer

Placement

  • Decide best clinical placement for client – taking into consideration infection control and diagnosis

Assessment

  • Preparing results and relaying to Doctor and uploading electronically for all medical staff to view
  • Receives updates of patient’s condition from handover (night to day) and the other members of the multidisciplinary team

Booking/Organising of Beds

  • Uses clinical knowledge and liaises with the whole multidisciplinary team regarding best placement of client
  • Brings all input from multidisciplinary team members together to make decision regarding discharge planning for client
  • Proactively accommodates next patients and ensures patients smooth and safe discharge

Transfer & Discharge

  • Brings all input from multidisciplinary team members together to make decision regarding discharge planning for client
  • Proactively accommodates next patients and ensures patients smooth and safe discharge
  • Ensure timely continuity of care by proactively outreaching to patients post discharge and facilitating transitions of care
Qualifications

• Registered at SANC
• Qualified as a Professional Nurs

Experience and Knowledge
• 3 years relevant experience. Sub-acute, palliative care, rehab, strokes, orthopedic, care for patient with IV therapy / chemo therapy or IV therapy
Skills and Attributes
• Computer literate, strong interpersonal skills, professional communication, ability to work under pressure, decision making ability, pro-ative, organisational skills compassionate, well presented, ability to converse with people on different levels – staff, families, MDT and other professionals coming into the facility.
 Working Hours
• Normal working days are Mondays to Fridays, from 07h00 until 14h00.

• Qualifications: Degree / Diploma / Certificate
• Copy of ID (If you are not a South African citizen please provide Passport and work/ study permit)
• New updated certificate and card from your council, e.g.: SANC. SASOHN, SAPC, etc. (should you have qualified outside of South Africa, please provide us with your registration number from the country you qualified in as well)
• Service Letters (if you have worked in a foreign country we will need these letters from the country you worked in.)
